Access and Functional Needs Registry
Linn County Emergency Management maintains a voluntary Access and Functional Needs Registry to help us better plan for and support residents who may need additional assistance before, during, or after an emergency. Information collected through this form may include contact information, address or location, communication needs, mobility or transportation needs, medical equipment or power-dependent devices, caregiver information, service animal information, and other support needs that could affect emergency planning, evacuation, sheltering, or response.
This information helps emergency management, public safety, public health, and response partners identify potential resource needs and improve life safety planning during incidents such as severe weather, flooding, extended power outages, evacuations, or other emergencies. Registration is voluntary, does not replace a personal emergency plan, and does not guarantee assistance during an emergency. Information submitted through this registry will be used for emergency preparedness, response, and recovery purposes and will be managed consistent with applicable privacy, records, and data security requirements.
